Why it usually happens
A campaign link at batch or unit level that happens to overlap a recalled lot. The realistic case is not carelessness — the person adding a campaign link is not the person watching the recall.
What to do
Point the new link at grains outside the recall's scope, or wait until the recall is lifted. Either way the notice keeps winning the scan in the meantime.
